When you first see a positive pregnancy test, one of the very first things you do is look for a pregnancy due date calculator. These interactive online tools promise to pinpoint the exact day your baby will arrive. But how accurate are they really, and what does the medical research actually say about how babies choose their birthday?
According to clinical studies and leading medical groups like the American College of Obstetricians and Gynecologists (ACOG), only about 5% of babies are born exactly on their estimated due date (EDD). The remaining 95% arrive either earlier or later, with the vast majority delivering within a two-week window before or after the target date.

The Standard Method: Naegele’s Rule
Most online due date calculators rely on a formula known as Naegele’s Rule, which was established in the early 19th century. The calculation is simple:
- Identify the first day of your last menstrual period (LMP).
- Subtract 3 calendar months.
- Add 7 days.
- Add 1 year.
For example, if your last period started on September 10, subtracting 3 months gives June 10. Adding 7 days gives June 17 of the following year.
While Naegele’s Rule is a helpful starting point, it relies on several biological assumptions:
- Your menstrual cycle is exactly 28 days long.
- You ovulate exactly on Day 14 of your cycle.
- Your gestation period is precisely 280 days (40 weeks) from the LMP.
In reality, cycle lengths vary widely. Research indicates that only a small percentage of women have a perfect 28-day cycle, and ovulation can occur earlier or later depending on stress, health, and individual physiology. If you ovulate on Day 18 instead of Day 14, a calculator using Naegele’s Rule will overestimate your baby’s gestational age by four days.
What the Research Says: Conception vs. LMP
Due date calculators are highly sensitive to the inputs you provide. If you know your exact conception date (for example, if you tracked ovulation using basal body temperature or used assisted reproductive technology like IVF), the calculation is adjusted.
When calculating from conception, gestation is assumed to be 266 days. This is because the standard 280-day count includes the two weeks before ovulation when you are not yet technically pregnant.
For IVF pregnancies, due dates are exceptionally precise. ACOG guidelines state that the due date for a pregnancy resulting from in vitro fertilization (IVF) should be established using the date of the embryo transfer. For a Day 3 embryo transfer, the due date is calculated by adding 263 days; for a Day 5 transfer, you add 261 days.
Clinical Gold Standard: First-Trimester Ultrasound
Because menstrual cycles can be irregular and memory is not always perfect, doctors rely heavily on ultrasound measurements to confirm or adjust your due date.

It is important to remember that a due date is not an expiration date; it is an estimate. Gestation is a biological spectrum. Research published in the Human Reproduction journal shows that even among healthy, uncomplicated pregnancies, the natural variation in gestation length can span up to five weeks.
Factors that influence when your baby actually arrives include:
- Parity: First-time mothers are statistically slightly more likely to deliver after their due date (averaging 40.5 weeks).
- Maternal Age & Weight: Higher maternal age or body mass index (BMI) can sometimes correlate with longer gestations.
- Genetics: If your mother or sisters delivered past their due dates, you may share a genetic predisposition for a longer pregnancy.
